No ideas, but disease comes to mind. Anybody?
I had this happen to my smokehouse graft this year. Took and looked beautiful… A foot of growth and then blackened and died back completely. I believe it was a rootstock failure, as when I cut back the bark on the rootstock there was no sign of life.
This was a bench graft on antanovka
Could be also fireblight.

This is what I do but the idea is not mine, I read about using the splicing tape here on the forum. Since it was at least five years ago, I do not remember who was/were the original proponent(s).
